Health Care Directive


You have a right to stipulate treatment, procedures, or medication that you authorize or refuse to consent to, or direct to be discontinued, in the circumstances set out in your directive.

This power of attorney also contains a living will provision.  If you are suffering from a terminal condition and your attending physician and another physician independently of each other, have determined that there can be no recovery from such condition and your death is imminent, where the application of life-prolonging procedures would serve only to artificially prolong the dying process, this document provides that such procedures be withheld or withdrawn and you be permitted to die naturally with only the administration of medication or the performance of any medical procedure deemed necessary to provide you with comfort, care and alleviate pain. After you print out your health care directive you will be able to initial the life-prolong decisions you want or do not want.

You have the right to revoke the authority granted to your agent by a later directive, a later writing declaring an intention to revoke the directive, or the destruction, with intent to revoke, all original executed copies of the directive either by you or by some other person in your presence and at your direction.

Notwithstanding the above, this power of attorney reserves you the right to make health care decisions for yourself as long as you are able to give informed consent with respect to a particular decision.  In addition, no treatment may be give to you over your objection.

General Power of Attorney


You may appoint an agent to perform any act which you could personally perform.  If you want to limit the power of your attorney to a specific act or acts you may do so.   You can appoint one attorney or two attorneys to act on your behalf.  You can either require that they act together or permit them to act separately. 

You may declare in your power of attorney that it is to continue despite subsequent mental infirmity on your part.
